  1. S.O. Maternity Leave Computation ( Special Order )

  2. S.O. "The Return from Maternity Leave" (Special Order )

  3. Proof : a. Live birth - Birth Certificate

b. Miscarriage - Medical Certificate, Clinical Abstract, Decidua, Histopathology, etc.
